Care coordination is a multi-touch model where people opt-in to working with a 2-1-1 on a short-term basis to help them navigate their local system of care until they gain access to services. This model works well for specialty populations (veterans, homeless individuals and family, education, patients, etc.) needing more help than a single conversation can offer. We are available to consult with you about your care coordination needs and planning.
